Diabetes Service

We support adults and children with diabetes and women with gestational diabetes during pregnancy.

Our services

We offer a range of educational services for people staying at Caboolture Hospital including:

  • diabetes education to children and adults with type 1 diabetes
  • introduction of insulin for type 2 diabetes
  • diabetes educator and dietitian support to the antenatal clinic.

Our team consists of doctors, nurses, dietitians, educators, social workers, an exercise physiologist and a psychologist. The Community, Indigenous and Subacute Service (CISS) Diabetes Team provides diabetes education and care to Caboolture Hospital patients.

What happens when I leave hospital?

After discharge, patients can be seen at one of our Specialist Diabetes Outpatient Clinics closest to their home, this includes:

  • Caboolture Community Health Centre (paediatric diabetes and adult diabetes), McKean Street, Caboolture
  • North Lakes Health Precinct (paediatric diabetes and adult diabetes), 9 Endeavour Boulevard, North Lakes
  • Chermside Community Health Centre (adult diabetes), 490 Hamilton Road, Chermside

How to access this service

Inpatient referral only

No external referrals are accepted for the inpatient Diabetes service. You can only access this service when you are staying in hospital or if you have been referred by a hospital clinician.

GP or medical practitioner referral

To access the Specialist Diabetes Outpatient Clinic, your GP or medical practitioner will need to send a referral letter to CISS.

If there is a waiting list, you will receive a confirmation letter and be advised on what to do next. If there is no waiting list, you will receive an appointment booking letter or we will contact you to arrange a suitable time for your appointment.

What to bring

Each time you visit us
  • Your Medicare card
  • Private health insurance card (if you have one)
  • Health Care Card and/or concession card (if you have one)
  • Current medications (prescription, over the counter and herbal medicine)
  • Relevant x-rays, scans or any other test results or reports
  • Glasses, hearing and mobility aids
For an outpatient or clinic appointment
  • Your appointment letter
  • Any special items listed on your letter
  • WorkCover claim number (if relevant)
  • Snacks, a drink or money to buy refreshments
  • Something to read or do while you wait
