The Transitional Care for the Redcliffe – Caboolture Mental Health Service is a social work team that provides brief interventions and follow up to individuals who have been admitted to Caboolture Mental Health Unit and who are not currently under the care of existing mental health services.
Our services
The Transitional Care Service can provide support for up to 28 days after discharge from hospital. If required, consumers may be able to access a medical review by the treating doctor who they saw on the ward.
How to access this service
The Transitional Care Team will make an appointment to see consumers within 7 days of discharge at either the Caboolture Hospital or Redcliffe Adult Mental Health Service. It’s also possible to negotiate for appointments to be at home or another community agency. This service is accessed by an inpatient referral only.
