What to bring<\/h1><\/div>

Each time you visit us<\/h3>\n
    \n
  • Your Medicare card<\/li>\n
  • Private health insurance card (if you want to use it)<\/li>\n
  • Health Care Card and\/or concession card (if you have one)<\/li>\n
  • Current medications (prescription, over the counter and herbal medicine)<\/li>\n
  • Relevant x-rays, scans or any other test results or reports<\/li>\n
  • Glasses, hearing and mobility aids<\/li>\n<\/ul>\n

    For an outpatient or clinic appointment<\/h3>\n
      \n
    • Your appointment letter<\/li>\n
    • Any special\u00a0items listed on your letter<\/li>\n
    • WorkCover claim number (if relevant)<\/li>\n
    • Snacks, a drink or money to buy refreshments<\/li>\n
    • Something to read or do while you wait<\/li>\n<\/ul>\n

      For a day procedure<\/h3>\n
        \n
      • An adult carer to support you and take you home<\/li>\n
      • Copy of Advanced Healthcare Directive or Enduring Power of Attorney (if you have one)<\/li>\n
      • A jacket or cardigan in case you get cold<\/li>\n<\/ul>\n

        Staying overnight or longer<\/h3>\n
          \n
        • A dressing gown or cardigan, sleepwear or comfortable day clothes<\/li>\n
        • Slippers and shoes (low heel, closed-in, non-slip, adjustable and firm fitting around the foot)<\/li>\n
        • Your toiletries and personal items<\/li>\n
        • Copy of Advanced Healthcare Directive or Enduring Power of Attorney if you have one<\/li>\n
        • Something to read or do during your stay<\/li>\n
        • Medical devices required for your care (e.g. CPAP machine)<\/li>\n
        • Do not bring large amounts of cash or valuables such as jewellery, watches or other items of value to you<\/li>\n<\/ul>\n<\/div>
