Geriatric Outreach Assessment Service

We provide a specialist Geriatric Assessment Outreach Service to help elderly residents living in residential aged care facilities avoid unnecessary hospital admission.

Our services

Our medical and nursing team offer hospital-level treatment for a range of conditions including:

  • urinary tract infections
  • pneumonia
  • cellulitis
  • falls
  • chronic obstructive pulmonary disease
  • heart failure
  • constipation
  • acute behavioural changes/delirium
  • end of life support.

How to access this service

To access this service, your GP or medical practitioner will need to contact the Geriatric Outreach Assessment Service by phone and send a referral letter to the hospital.

Once the referral is received, our team will contact your residential aged care facility to set up an appointment to visit you within 24 to 48 hours depending on your condition. In most instances, you will be seen on the same day so you can receive appropriate treatment.

After your hospital appointment

The hospital doctor may advise ongoing treatment with intravenous medications, which will be provided by the Hospital in the Home Service. If further review is required, the team will organise another appointment to visit you at your residential aged care facility. Once your condition has improved and you no longer require hospital-level treatment, your care will be transferred back to your GP.
