LINK and SEED Innovation Funding Showcase

Projects aimed at improving the social support of vulnerable patients and ensuring patients and families are supported during end of life are among the highlights of this year’s LINK and SEED Innovation Funding Showcase.

Projects aimed at improving the social support of vulnerable patients and ensuring patients and families are supported during end of life are among the highlights of this year’s LINK and SEED Innovation Funding Showcase.

Metro North provides $1million through the LINK fund to build external partnerships and improve continuity and quality of care for patients. Another $1.1m is provided to SEED projects for Metro North staff to develop new and improved ways of delivering health services.

The showcased projects were part of the 2015 funding round.

LINK presentations included the Working Together to Connect Care project, which partnered with Micah and Footprints to identify and assist people in the community who frequently attend emergency departments; the GRACE (GP Rapid Access to Consultative Expertise), the Lymphoedema Clinical Pathway Project, Metro North Cancer Care Information Hub and Caboolture Hospital’s Integrated Care Pathway for Vulnerable Children program.

SEED presentations included the Kidney Supportive Care Program that focuses on quality of life for patients with chronic kidney disease and their families; Avoiding Emergency presentations for Cancer Patients Project, and the Central Venous Access Device (CVAD) Registry project.
