Rehabilitation care
We support adult patients of all ages who have care needs which require them to undertake intensive therapy program to help improve general function and independence. We work with patients who:
- have neurological conditions, including stroke and Parkinson’s disease
- have orthopaedic conditions of the upper and lower body, including hip or knee replacement surgery, lower limb amputation or recovery from fractures
- are suffering from a prolonged illness, multiple injuries or respiratory conditions
- are recovering from an acute illness or injury, and
- would benefit from a period of rehabilitation.
Our services
Patients work with a team of healthcare professionals including:
- doctors
- nurses
- physiotherapists
- occupational therapists
- pharmacists
- social workers
- dietitians
- speech pathologists
- neuropsychologist.
Patients have access to multidisciplinary services that will support their rehabilitation and improvement in functional capacity and enable them to return to their home environments or other discharge destinations such as residential care facilities.
How to access this service
Referrals can be made by the patient’s acute treating team or in public hospitals by the Geriatric Rehabilitation and Liaison Service.
