Referral form – Alcohol and Drug Information Service

Alcohol and Drug Information Service (ADIS) is a 24/7 non-clinical service for people whose lives are affected by alcohol and/or drug use and their families. In response to this referral, an ADIS clinician will call the client as soon as possible and provide support appropriate to the individual.
