    Our service assists people aged between 18-65 years old who have a complex mental illness.

    Our services

    We help consumers to understand their illness and ways to reduce or cope with symptoms.

    We help guide consumers and their support people (family, friend or carer) to groups in the community to enhance their quality of life. There are a number of specialist services available including healthy living groups, anxiety management and education forums.  Our consumer and carer workforce have a lived experience of mental illness and can offer support during a consumer’s recovery journey.

    The treating team may include a doctor, psychologist, nurse, social worker and occupational therapist.

    How to access this service

    Referrals are accepted from the Acute Care Team, Homeless Health Outreach Team, Inpatient Services and the private sector. The Continuing Care Team will assess the referral to determine the most appropriate service for you. There is no waiting list to access this service.

    What to bring

    Each time you visit us
    • Your Medicare card
    • Health Care Card and/or concession card (if you have one)
    • Recovery goals and plan
    • Medications you are currently taking, including any that you have bought without a prescription and herbal medicine
    • Private health insurance card (if you want to use it)
    • Glasses, hearing aid, walking frame
    For an outpatient or clinic appointment
    • Your appointment letter
    • Any special items listed on your letter
    • Snacks, a drink or money to buy refreshments

    Frequently asked questions

    Your doctor will see you every 6 weeks and your mental health clinician will see you every 2 weeks but this may change depending on your needs.

    We encourage you to involve your support person to give you extra support. Please let your mental health clinician know if you would prefer not to have your support person involved.

    Medication can be an important part of your recovery. Speak to your doctor or mental health clinician if you have concerns about your treatment.

    We are happy to answer any questions you might have. Please ask your mental health clinician or doctor.
