Community based rehabiliation referral form

Community based rehabilitation (CBR) provides services that aim to improve people’s wellbeing, functional capacity, independence and quality of life. People who will benefit from this service are those who have had a significant recent health event or injury and have the capacity to benefit from active participation in rehabilitation. CBR provides time limited services and programs from four, eight or twelve weeks.

GP’s can refer patients over 18yrs with complex conditions who would benefit from targeted, time limited interventions. Services delivered by a multidisciplinary team and may include:

• Physiotherapy
• Social worker
• Therapy assistant
• Occupational Therapy
• Speech pathologist

Essential information required

• Medical history including medications.
