Complex Chronic Disease Team (CCDT)

We provide a range of services for people who are at risk of admission or readmission to hospital or presentation to emergency departments. We support people who need a team of health professionals to manage their complex care needs and chronic disease.

Our services

We have an extensive team of health professionals providing specialist care including:

  • dietitians
  • exercise physiologist
  • occupational therapists
  • physiotherapists
  • psychologists
  • social workers
  • speech pathologists
  • nurses
  • doctors.

We also offer group rehabilitation programs for cardiac and pulmonary rehabilitation.

How to access this service

To access this service, your GP or medical practitioner will need to send a referral letter.

If there is a waiting list, you will receive a confirmation letter and be advised on what to do next. If there is no waiting list, you will receive an appointment booking letter or we will contact you to arrange a suitable time for your appointment.
