Interim Care (Ashworth House)

We support older people who require long-term accommodation in a residential aged care facility. The 28-bed unit provides support for up to 4 weeks. During this time we help patients get back to daily living activities as well as finalise long-term care arrangement.

Our services

We help patients make informed choices about their future accommodation needs. We aim to maintain and optimise the individual’s functional capacity with a multidisciplinary approach to:

• activities of daily living and ability to manage personal cares
• mobility and physical capacity maintenance or improvement where feasible
• continence
• leisure skills
• medication management
• communication
• diet and nutrition
• pain management
• education on chronic diseases
• preparation for discharge to a residential facility.

How to access this service

Referrals to interim care can be made by any member of the patient’s hospital treating team and must be assessed or deemed to be eligible by the Aged Care Assessment Service. Patients must have consented to the Residential Aged Care Pathway.
