Chermside Community Health Centre

The Chermside Community Health Centre provides a range of services for people living in the northern suburbs of Brisbane. The centre is located on the campus of The Prince Charles Hospital near the Hamilton Road entrance.

• Community Based Rehabilitation Team (CBRT)
• Diabetes Service (adults only)
• Post Acute Care Service (PACS)
• Indigenous Health Services
• Alcohol and Drug Service
• Melaleuca Clinic (opiate replacement therapy)
• Complex Chronic Disease Team (CCDT)

We also offer the following mental health services:

• Resource Team
• Older Persons Mental Health Service (OPMH)
• Community Mental Health Service (CCMHS)
• Mobile Intensive Rehabilitation Team (MIRT)
• Indigenous Mental Health Services

Parking and transport

Free car parking for clients is reserved at the front of the centre, you enter from Hamilton Road. A public bus service arrives and leaves from the front entrance of The Prince Charles Hospital.
